Hi ..would it be possible for you to print this paperwork I have filled out for
Jeffrey and get him to Read and Sign all the pages..?! His doctor appointment is Wed. July
11th at 8:45am...I will not see him before the appointment. I need to get this signed and sent
back over to them tomorrow...I have sent to him as well...Appreciate your help!! :) Lesley
