Pm writing to let you know what the supporting documents for your Employment Authorization Application are.
USCIS need to receive these within 10 days after the electronic filing of your application, so we need to mail
them by next Monday (02/04.) You can mail the documents to our office address (below) or bring them in
person. Please let me know which you prefer.
Supporting Documents:
1- Copy of your passport: Personal information pages and US visa pages (I have a copy but it's a bit fuzzy, I don't
want this to cause any delays. Could you make new copies?)
2- Copy of your I-94 document- both front and back (This is the white card that they attached to your passport at
the airport when you entered)
3- Copy of your last EAD (I have this, you don't need to send.)
4- Two color passport pictures
5- Copy of the receipt notice for your I-589 application. (I have the referral notice for the form 1-589, so no
worries if you don't have a receipt notice. But if you do have it, please send.)
