ITEM # 1, COMPLETE CONSTRUCTION OF ONE (1) ASPHALT TENNIS COURT, 60' X 120'.
Work to proceed as follows:
1. We receive site with completed asphalt pad, 60' X 120', meeting ASBA specifications.
2. Furnish and install one pair of Douglas 3" OD tennis net posts in sleeves set in concrete. Furnish
and install one Douglas center anchor.
3. Furnish and install 8000 Watt, 4 pole, black lighting system. Manufacturer to be RLS. Light pole
Footings to be supplied by others.
4. Furnish and Install 240 LF of 10' high, 1%", KK, black vinyl coated chain link fence.
5. Furnish and install 120 LF of 4' high, 2", KK, black vinyl coated chain link fence.
6. Furnish and install 2, 4' X 4' walk gates.
7. Furnish and install one complete OmniCourt Turf system. System includes all incidentials.
8. Tennis court to be green with sandstone colored sand. Lines to be white.
9. Furnish and install one Douglas Pro 645 BB system, with complete half court blended striping.
10. Furnish and install one top quality Douglas , HD tennis net complete with center strap.
11. Total job site clean up.

CONDITIONS
1. Signature of owner on accepted line of proposal, binds owner (LSJE, LLC.) and
Contractor (EvergreenTennis Courts, Inc.), to all the Items and Conditions of this proposal.
2. Prices quoted includes completion of all items on page one of this Proposal.
3. All materials and workmanship are guaranteed for a period of one year. OmniCourt is guaranteed
for five (5) full years.
4. The customer agrees to furnish access to site for equipment and materials.
5. The customer agrees to furnish room and board for 2 man crew. Laborers (3) supplied by owner at
No cost to contractor.
6. All materials shipped to Miami. Additional container shipping from Miami to jobsite, provided and
paid for by owner.
5. The customer agrees to furnish a clean water supply and electricity within 100' of job site.
6. No responsibility is accepted by contractor for damage or acts occasioned by anyone other than
contractor or its employees.
7. Evergreen Tennis Courts, Inc. is not responsible for delays caused by weather or other
unforeseen acts which it has no control over.
8. No work will be scheduled without written acceptance of proposal with required down payment.
EXCLUSIONS
1. Price does not include bond, permits, fees, traffic control, testing or landscaping.
TERMS
1. Customer agrees to a down payment of $ 75 000.00 due with returned copy of signed Proposal.
2. Customer agrees to a second payment of $ 25 000.00 upon delivery of materials to jobsite.
2. Customer agrees to a final payment of $ 22 500 00 upon completion of the Project.
SPECIAL CONDITIONS
1. If customer does not pay as agreed upon, and it becomes necessary to employ the services of an
attorney to secure payment, then the customer shall be liable for all said attorney fees plus all costs
of collection
2. All accounts that are more than 30 days past due, will be charged a $ 15.00 late billing fee plus 1.5%
interest per month.
