1) demo 46 4' x 4' concrete pads that are cracked or chipped @ driveway both
EAST and WEST and haul to dump.
2) Form and pour 46 4' x 4' concrete pads with fiber mesh and wire @ prepped
areas of drive to repair back to normal.
3) After 10 days of curing time we will then install a 1/2 " of topping that matches
existing surface.
TERMS: All surfaces will be prepped to Palm Beach Code and compacted to specs.
All toppings will match to the best professional match we can.
TOTAL: $12,565.55
PALM BEACH CONTRACTING SERVICES INC hereby proposes tofurnish materials as shown as stated above. All work is
guaranteed to be completed in a workmanlike manner according to standardpractice. Any changes inmiring extra costs or
extension of time will be executed upon written order and will become an extra change order over and above this proposal.

required to enforce the terms and conditions of this contract.
