Please FedEx the check to us ASAP since we need to receive prior to
appointment date given that it is in a few days.
Alternatively, you can call our front desk a and arrange for
credit card payment.
I will email you with the details of time and location but it would be at
either one of the locations:

I will email you by tomorrow to give you exact time and details.
If she has any prior records and testing please have her bring a copy. If
she has no prior testing records, no problem.
