Dr. Merrell's office is sending you a new patient form to fill out...be on
the look out-I told the gal who answered the phone that Jeffrey and Dr. Merrell
are friends and Jeffrey will want you to have an appt sooner rather than
later...The gal said to just reference that on the form and get it back to her
quickly and they will get you in-confirm you rec'd this and the email from
